When Viola gets sick she initially assumes she has the plague, but when a doctor comes to check he shows that she actually has the lungs, which he thinks has a very poor prognosis.
Most likely, based on Viola’s symptoms and description of the disease, the lungs are referring to tuberculosis, which was a very common disease at the time, was not easy to treat, and was essentially considered a death sentence.
